  office records. Dr. Miller DOES NOT do disability or "IME" disability
  exams or fill out lengthy disability forms.
· Please allow at least one week for your records request to be processed.
· Dr. Miller's office takes all steps necessary to protect your health records.
· Your information is only legally shared with your permission or signature
  if it is required for treatment by another provider or for payment from
  an insurance company.
